This form MUST be completed by all instructors & will be used to list your class on our website.

If you do not want your class publicly listed, please indicate that at the end of the form and your answers will be kept for internal purposes only. Fields marked with * are required to make sure your form is connected to your class. Questions? Email tanya@seb.org.
